Navajo School Clothing and Supplies Application
2026 School Year
The NDCFS Financial Assistance Unit (FAU) provides financial assistance for eligible school age (3-18 years old) Navajo children to purchase clothing and supplies, of their choice, in an effort to promote school attendance, encourage academic performance, and support self-confidence.
Please note that eligibility does not guarantee assistance as limited funds are available for NSCS.
Required Documentation
Before you begin filling out the application, gather the required documents and be prepared to upload an electronic copy as part of this application (photos of physical documents are acceptable).
To determine your eligibility for assistance, verification is required for the items below. All documents must have matching names on CIB, Social Security Card, W-9, and State ID/Driver’s License. If you do not provide all required documentation, your application will be denied.
Required documentation MUST be provided for BOTH Parent/Guardian and Students:
- Driver’s License/ID – Applicant only (Parent/Legal Guardian)
- Social Security Card – Parent/Legal guardian AND Child(ren); names must match this application
- Certificate of Indian Blood (CIB) – Parent/Legal Guardian (if Navajo) AND Child(ren)
- School (or Learning Enrichment Program) Enrollment Verification – obtain from School
- Legal Guardianship / Power of Attorney – Foster/Legal Guardian Parents ONLY
- If your name has changed, a marriage license or divorce certificate is required – (Applicant)
- Current W-9 Form – Parent/Legal Guardian (After submitting this application, you will be re-directed to fill out a W-9 Form online; your name must match on this NSCS application, the W-9, and required Social Security Card.)
